We don't really have any control over what custom bars or buttons have been configured. We did recently add more icons so that toolbars take less space. are you sure the icons are not there?

You can use the tools menu to export your settings after making changes. Then you can restore the settings if the problem happens again. It's a quick operation and also allows you to be quite specific about which settings you export.

You can add commands to any toolbar, vs might prefer if you add the commands to a built in vs bar.
